A "transition" (in the context of career/lifework planning) is the planned or unplanned change in one's career/lifework. A planned transition is considering, planning for, looking for, and finding a new job.
An unplanned transition is losing, changing or modifying one's job due to an unexpected change in one's life (illness, death in family, etc), being fired or terminated.
