If you are producing your own chapter to be merged into a monograph or book, you must first define a reference caption of "Appendix". In MS Word go to Insert, then "Reference", then "Caption", then type "Appendix" in teh box to the right of "Label".

How do I refer to an Appendix within a paragraph so the title and page number appear?
